Assessment & Report Services

Individual Assessments

Each assessment is carefully designed to address the specific concerns identified by your family and/or your child’s educators or other support persons. Each assessment is tailored to meet your unique needs and conducted in partnership with you. The assessment process may take place over several sessions depending on the areas of focus. Depending on the assessment needs, it may include: 

  • A caregiver interview 
  • The administration of standardised and/or non-standardised occupational therapy assessment tools. 
  • Observation of the child or young person in their naturalistic environment 

At Bloom Paediatric and Family Therapy, I am passionate about empowering caregivers and families to advocate for their child or young person’s needs. This often includes providing comprehensive written documentation to support advocacy efforts. 


Below are the types of the more specific assessments and reports available:

VCE Handwriting Assessment & Report

For students applying for special consideration in their written VCE exams and assessment tasks, I offer a detailed handwriting assessment and report. This report can be submitted to VCAA as part of your application to demonstrate your child’s specific needs.

Disability & Inclusion Profile (DIP) Funding Application Report

 If your child or young person’s school is applying for DIP funding, I can provide a thorough assessment and report that highlights evidence of the required level of support under each domain of the Disability and Inclusion Profile.

Kinder Inclusion Support (KIS) Application Report

For families working with kindergartens applying for additional support, I can collaborate with your child’s educators to develop a KIS application report. This report provides evidence to support the funding required to enable your child to participate in the kinder setting to the best of their abilities. 

Transition Reports

Transitions can be significant milestones in a child or young person’s life. A transition report offers a detailed introduction to your child, highlighting their strengths, areas of support, and specific recommendations for a smooth transition. These reports can also be prepared at the end of a school year to introduce your child to their next teacher and provide tailored strategies for support.

NDIS Reports

NDIS Functional Capacity Assessment (FCA) Reports

For a more in-depth evaluation of your child or young person’s functional abilities and support needs, FCA reports include multiple occupational therapy specific assessments, caregiver and child interviews, and evidence-based recommendations for future supports.


NDIS Supporting Evidence Reports

These reports provide detailed evidence across all NDIS functional domains to support your initial funding application with the National Disability Insurance Agency (NDIA).


NDIS End of Plan Reports

These annual reports summarise services provided, progress towards NDIS goals, and recommendations for your child or young person’s next NDIS plan.
